Please complete this form in its entirety and either present it to the IGA on the date of the auction, but prior to placing or accepting any bids, or mail/fax it to the following:

I hereby appoint _______________________________, to act as my authorized agent for the 2006 Iowa Greyhound Association Auction to be conducted on May 20, 2006, at the Iowa State Fairgrounds.
As my agent, I understand that he/she is authorized to represent me in all matters pertaining to the sale or purchase of any consigned greyhounds, litters, and/or equipment/services offered for sale at the 2006 Iowa Greyhound Association Auction. I further understand that the above identified agent is responsible for tendering payment for any lots purchased as required by the Terms and Conditions established by the Iowa Greyhound Association.
I agree that this Authorization shall be revocable only in writing, such revocation to become effective when acknowledged in writing by the Iowa Greyhound Association.
